Premium Health is looking for outstanding candidates for the Patient Engagement Specialist position to support and assist in the implementation of care optimization across multiple departments to ensure continuity of quality patient care.

The care optimization department has multiple functions relating to the continuum of patient care. These functions bridge the gap between Patient and Doctor and improve community health outcomes. The ideal candidate will use our C.A.R. E. standards of Compassion, Agility, Respect, and Excellence as a guide for optimal patient care.  

Suitable candidate should have a proactive, can-do attitude and work or volunteer experience in health, education or related field. Strong verbal skills and High school diploma required. Knowledge in Excel a plus.
